SPECIMEN REFERRAL FOR SECOND OPINION

The cytopathologists at The Johns Hopkins Hospital are available for consultation on diagnostically challenging cytologic specimens.

The slides should be sent to the Division of Cytopathology (address below) along with the completed form, clinical information and pathology slides.

CONSULTATION BILLING AND REPORT INFORMATION

Due to the large volume of pathologic slides referred to The Johns Hopkins Hospital and the wide variety of reasons for such referrals, we must request that the following form, with all of the indicated information, accompany the slides and reports. Failure to complete and submit the form may result in delays in patient care and incomplete or inappropriate communication with the responsible and interested physicians.

This form, the appropriate clinical information and pathology reports must accompany slides referred for any reason to The Johns Hopkins Hospital or any member of the staff.
