          - Provide two tubes: one Lavender Top Tube: collection volume 4 mls (min 2 mls) and one Green top: collection volume 4 mls (min 2 mls)
          - Room temperature (preferred), may be refrigerated - DO NOT FREEZE
          - Turnaround time: 4 weeks

     Tissue
          - Skin biopsy in provided transport media; keep refrigerated - DO NOT FREEZE
          - Call the laboratory for a skin biopsy kit
          - Turnaround time: 4 - 6 weeks

     Other
          - Call the laboratory
